Patient Registration

You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. On your first visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

Directions
Lowell Office: North or South: From Route 495 take Route 38 (Exit 38). Take a right off the highway onto Rte 38 North. Follow Rte 38 North for approximately 2 miles (following the signs for Saints Medical Center). At the set of lights for the hospital, take a left onto East Merrimack Street. Take your 1st right onto Saints Medical Center Campus. The parking garage is on the right. The office is located in the Brendan Leahey Medical Building on the 4th floor.

Nashua office: Follow Route 3 North to exit 8. Follow the exit to the very end where you will see Bertucci's in front of you. Turn left at the light onto Amherst Street (Route 101A). Turn right at the next light (McDonald's is on he corner) onto Cotton Road. Once you are on Cotton Road, bear right towards the office park. We are the second building on the left. We are located at 20 Cotton Road on the 2nd floor.
